I am the lawful parent(s) and/or guardian(s) of the above-named child(ren), and I hereby consent to the participation of my child(ren) in the activities curriculum, programs, and care conducted by Rosewood Park Alliance Church and its staff and authorized volunteers.
I understand that there are inherent risks, dangers, hazards, and liabilities to all participants participating in the Rosewood Park Alliance Church day camp. Rosewood Park Alliance Church and its directors, employees, members, and volunteers are not responsible for any injury, personal injury, damage, property damage, expense, loss of income, or loss of any kind during, or as a result of, participating in any events with Rosewood Park Alliance Church.
I acknowledge that it is my sole responsibility to notify, inform, and update Rosewood Park Alliance Church of any medical conditions of my child(ren), including but not limited to known drug and food allergies, known dietary restrictions, and other medical conditions. In the event of a medical emergency, I understand that every reasonable effort will be made to contact the parent(s) and/or guardian(s). I hereby give permission to the Rosewood Park Day Camp staff and volunteers, licensed emergency, and health care personnel to provide treatment/services they deem necessary with respect to my child(ren). I agree that all medical information provided by myself as the parent/guardian of the child(ren) to Rosewood Park Alliance Church may be disclosed to such emergency and health care personnel. In the event of illness, accident, emergency, or any other circumstance requiring medical treatment, such treatment may be procured for my child(ren) without legal or financial obligation to Rosewood Park Alliance Church.
Rosewood Park Alliance Church provides a safe and nurturing environment for our campers, volunteers, and staff. Respect and trust are the most important elements of our camp experience. We ask that before you send your child(ren) off to camp, please talk to them about respecting others, the rules, and the church facilities.
Rosewood Park Alliance Church reserves the right to contact the parent/guardian of any camper who refuses to respect other campers, volunteers, or staff. Abusive and/or inappropriate language and actions, harming others, and destruction of church property are not tolerated. Rosewood Park Alliance Church reserves the right to dismiss any child or student that violates the rules of conduct for day camp with no refund of registration fees.
